We welcome you to our practice and appreciate that you have selected our dental team to provide you with excellent dental health. We take pride in and are committed to providing you quality oral health care in a comfortable, gentle, and professional environment. We look forward to becoming a partner in your health care.
During your initial visit, a thorough examination will be performed. It will include the appropriate x-rays, periodontal screening, oral cancer exam, and blood pressure check to assist in making an accurate diagnosis of the current health of your mouth, teeth, and gums. We strive to maintain and retain the health of your natural teeth. By working together to develop a mutual understanding and clarify our expectations of one another, we will reach that goal. Once an appropriate treatment plan has been determined, the estimated fees will be presented to you. If treatment is indicated, our goal will be to restore optimum dental health in as few, well-planned visits as necessary.
We understand the value of your time and are confident you will return the courtesy. You can expect us to be on schedule for you, except for emergency situations that may arise during the course of the day. We make every effort to notify our patients of changes or delays in the daily schedule as timely as possible. Should you need to reschedule your appointment, please keep in mind there may be other patients who need or have requested an earlier appointment; therefore, we require a 2 business day notice, this will also ensure that charges will not be incurred due to lack of advance notice.
On this website you will find our Welcome Letter/Patient Information form, Financial Consent form and Privacy Notice forms. Please complete these forms and bring them to your first visit. If you have dental insurance, please call the office prior to your appointment to give us your insurance information so we can verify your benefits. We would also like to ask you to bring your benefit identification card to your appointment so that we can assist you by filing your dental claims. Please check with your insurance carrier prior to your appointment regarding out of network benefits and whether services at Sandy Point Dental are covered under your policy. We do accept assignment of benefits from insurance carriers that allow you to visit an out of network provider. We are also currently working in network with all Delta Dental Premier and PPO plans. You are responsible for any balances unpaid by your dental insurance carrier. Payment or insurance co payment is expected at the time services are rendered for all patients with or without insurance. Methods of payment include Visa, Mastercard, Discover, check or cash.
